Compassionate Palliative Care referrals for GPs and specialists
Karuna Hospice Services partners with GPs and medical specialists to provide compassionate, home-based palliative care for people living with a life-limiting illness.
Our multidisciplinary team works alongside the referring clinician to support patients and their families with specialist palliative nursing care, symptom management, and psychosocial and spiritual support. The person’s primary medical care remains with the patient’s GP or treating specialist.
Who can refer
This form can be completed by:
- GPs.
- Medical specialists.
- Hospital and community health professionals.
How referrals work
- Call (07) 3632 8300 (Monday–Friday, 8am–4pm) To speak with our referrals team.
- Complete the Karuna Patient Referral Form (link) and attach relevant supporting documentation.
- The referred person will be contacted directly to arrange the first in-home admission visit.
- The referrer will be kept informed of the proposed care plan and progress.
Eligibility and costs
Our at-home palliative care services are best suited to people who:
- Have poor or deteriorating health because of one or multiple life-limiting conditions, despite treatment and management.
- Wish to focus on quality of life, strengthening their physical, emotional, social and spiritual well-being.
- Are connected to a GP or specialist doctor.
- Have consented to a referral to Karuna for in-home support and/or by their appointed decision maker.
- Reside within the Karuna catchment area of Brisbane central/north, Redcliffe, Caboolture and/or Bribie Island* (refer to map below).
People referred to Karuna who live within the catchment area (Brisbane central/north, Redcliffe, Caboolture and Bribie Island) receive care at no cost.
If the person resides outside this catchment area, services may still be available, however a fee may apply.
