Personalised Palliative Care planning
The first step when you engage with Karuna is a thorough, person-centred assessment of your unique situation.
Our specialist palliative care team, including our doctors, nurses and social workers, will conduct a holistic assessment with you and your loved one of their needs, your needs as a carer, and the overall situation at home. This process leads to developing together the best plan for care designed to support you and your loved one to live well and die peacefully in their place of choice.
This plan addresses a range of needs, including:
- Safety and practical assistance: We consider in-home support, equipment hire, and necessary modifications. We also work closely with external home care providers and resources, such as My Aged Care, for assistance with daily tasks like showering, cleaning, and maintenance.
- Management of complex symptoms: Our team focuses on comfort, symptom control, and maximising quality of life.
Holistic guidance: We look beyond the clinical to include emotional, social, cultural, and spiritual needs based on your unique beliefs, values, and comfort levels.
We know your needs may change over time. Karuna will be there to adjust how we support you as the carer, alongside the person receiving care – including symptom management, medication, counselling and spiritual care and social work support. This ensures the care you each receive remains aligned with what is important to them, and to you as their carer.
Sometimes it’s possible that caring at home is no longer an option. If that is the case, we can also suggest alternatives and support you and your family.
